Form Templates (Tier II & III)
All Tier II & III Programs must complete a Consent to Participate form and collect from all participant families no later than the program's start date. Templates are available in the .
Required items include the following:
- Assumption of Risk & Informed Consent to Participate
- Behavioral Expectations & Code of Conduct
- Permission to Transport
- Photo & Video Consent & Release
- Release for Emergency Care
The above mentioned documents must be retained in accordance with the (Current year + 6).
